Explore Torrance Memorial Medical Center’s Community Care Resource Directory, designed to support families seeking trusted home-based services throughout the South Bay and surrounding communities. This comprehensive directory includes three essential resource lists: non-skilled Home Care agencies offering companion care, personal care, homemaking, transportation, and live-in support; Medicare-certified Home Health agencies providing skilled nursing and therapy services following hospitalization or physician referral; and licensed Hospice and Palliative Care providers focused on comfort, dignity, and quality of life. Each list includes verified contact information and service categories to help families make informed decisions during important transitions in care. While agencies are not endorsed, this centralized resource simplifies the process of identifying appropriate support options. Whether you are planning ahead or responding to an immediate need, this directory offers clear, organized access to community care services when it matters most.

Description

Community Care Resource Directory

When families are navigating health care decisions at home, having clear, reliable information matters.

Torrance Memorial Medical Center provides this Community Care Resource Directory as a courtesy to help individuals and caregivers identify home-based support services throughout the South Bay and surrounding communities. This directory brings together three essential resource lists in one convenient location:

  • Home Care Agencies – Non-skilled in-home support services such as companion care, personal care assistance, homemaking services, transportation, live-in care, and care/case management.

  • Home Health Agencies – Medicare-certified providers delivering skilled medical services in the home, including nursing care and therapy services following hospitalization or physician referral.

  • Hospice & Palliative Care Providers – Specialized care focused on comfort, dignity, and quality of life for individuals facing serious or life-limiting illness.

Each list is maintained to provide current contact information and service categories. Inclusion on these lists does not imply recommendation or endorsement. Families are encouraged to conduct their own research and speak directly with agencies to determine the best fit for their needs.

Our goal is simple: to support informed decision-making during times when clarity, compassion, and trusted information are especially important.
