A message to our Torrance Memorial patients needing Medical Records
At Torrance Memorial, the health and well-being of our patients, our employees,
and our community is our top priority. We understand the concern and uncertainty
you may be experiencing surrounding the coronavirus (COVID-19). We remain
committed to being responsive to the needs of our patients as the situation
continues to evolve.
We are strongly encouraging you to use the
MyTorranceMemorial patient portal to conveniently access your medical records, which includes your physician
reports, procedure reports, allergy history, home medication list, immunization
history, and view your test results. You may also review your discharge
instructions, the summary of your care, and the discharge educational
materials. You may also send your medical records to your physician’s
office and send a secure message to Torrance Memorial. If you haven’t
created your MyTorranceMemorial account,
enroll now.
Release of Health Information

How to Make a Request
To obtain a copy of your medical records from Torrance Memorial, please
download the form below. You may also use this form to have your medical
records sent to another medical facility.
Processing a Request
Records are usually available within 7 to 10 business days from the time
the request is received. If you are interested in picking up your medical
records in person, please note this at the top of the request form. If
you plan to come to the Medical Records department to pick up a request
that you have submitted, you must have a photo ID with you. To arrange
for another individual to pick up the documents for you, please contact
the Health Information Department for instructions at 310-517-4721.
Once your "Authorization for Use or Disclosure of Protected Health
Information" form is completed, you may submit your request by mail,
fax, or in person.

Completing an Authorization
The Health Insurance Portability and Accountability Act, also known as
HIPAA, requires hospitals to obtain an original, complete, and properly
executed "Authorization for Use or Disclosure of Protected Health
Information" form in order to provide a copy of a patient's medical
record to a patient or anyone he or she designates. Only the patient,
parent/legal guardian, or the patient's legal health care representative
can sign the form to release medical records.
The authorization form must be legible and complete. Authorizations that
are illegible or incomplete will be returned. Requests for information
regarding any of the following items must also be specifically initialed
on the form:
- AIDS/HIV
- Mental Health
- Chemical Dependency
Authorization to Release Forms
Please note that the authorization is valid one year from the date signed
for the specific visit that you request.
Birth Certificates
Birth certificates for individuals born at Torrance Memorial may be obtained
from the Los Angeles County Office of the Clerk Recorder. Visit its web site at
LAPublicHealth.org (click on vital records) or you may call 213-240-7812 for more information.
Torrance Memorial does not keep or maintain birth certificates.
Radiology and Imaging Results Request
Radiology and imaging results requests can now be obtained from Medical
Records. You can call (310) 517-4721 or visit Medical Records in-person
in the West Tower, 1st Floor.
Billing Information
Please note that your medical records do not contain any billing information.
For questions about your Torrance Memorial bill, please call the Business
Office at 800-661-0004.
Fees
Records released to a patient, physician's office or a medical facility
are free of charge.
