At Torrance Memorial, the health and well-being of our patients, our employees,
and our community is our top priority. We understand the concern and uncertainty
you may be experiencing surrounding the coronavirus (COVID-19). We remain
committed to being responsive to the needs of our patients as the situation
continues to evolve.
We are strongly encouraging you to use the
MyTorranceMemorial patient portal to conveniently access your medical records, which includes your physician
reports, procedure reports, allergy history, home medication list, immunization
history, and view your test results. You may also review your discharge
instructions, the summary of your care, and the discharge educational
materials. You may also send your medical records to your physician’s
office and send a secure message to Torrance Memorial. If you haven’t

How to Make a Request
To request records and Radiology images from the hospital (Torrance Memorial
Medical Center), complete the online form below. You will be required
to provide a valid email address and a Government issued ID.

- Your records will be ready in 5 ‐ 10 business days from the receipt of
your electronic request.

Physician's office records
To request records from my doctor’s office or urgent care (Torrance
Memorial Physician Network), click
If you are unable to complete the electronic form above, download and complete
the paper form below. The authorization form must be legible and complete.
Authorizations that are illegible or incomplete will be returned. For
requests with sensitive information such as AIDS/HIV, mental health, and/or
chemical dependency, use the Authorization for Use or Disclosure of Sensitive
Protected Health Information form. The authorization is only valid for
the specified date requested and expires one year from the date signed.
Please note the processing of records is faster using the online form above.
Records are usually available within 10 business days from the time the
request is received. If you are picking up your medical records in person,
please be sure to bring a government-issued ID. To arrange for another
individual to pick up the documents for you, please indicate on the authorization form.
Only the patient, parent/legal guardian, or the patient's legal health
care representative can sign the form to release medical records. If you
are requesting records on behalf of the patient or as the patient’s
representative, please provide a copy of an Advance Directive/Durable
Power of Attorney for healthcare/Conservatorship.

from the Los Angeles County Office of the Clerk Recorder. Visit its web site at

Torrance Memorial does not keep or maintain birth certificates.
Please note that your medical records do not contain any billing information.

Records released to a patient, physician's office or a medical facility
for treatment purposes are free of charge.
