Torrance Memorial Home Health Care offers quality health care in the place people are most comfortable — their home. Our professional team of nurses, therapists and other staff provides personalized attention and cost-effective care, allowing patients to remain as independent as possible.

Home health care is for patients who require skilled intermittent care. Additionally, we coordinate home health care with other programs at Torrance Memorial, including palliative care and hospice.

When You Need Health Care at Home

We know you are looking forward to going home. After all, home is a familiar environment, and family and friends are close at hand to participate in a more direct role in your care. We recognize how important this can be to your recovery process.

In fact, we begin before you even leave the hospital. We will contact your insurance carrier to assist you in determining your coverage, and pre-authorize treatment, if possible. Our discharge planning nurse or home care coordinator works closely with your doctor to develop a treatment plan specifically designed for you.

Once you are home, we do individualized training with both you and your family to ensure that your treatment is properly administered. Depending on your needs, our nurses will coordinate the support of other services and treatments including medical social workers, physical and occupational therapists, speech pathologists, pharmacists, and nursing attendant services.

Home Health Heart Failure Program

Home Health has a Heart Failure program to educate patients on their disease, medication management and recognition of early warning signs in order to maintain the patient safely in their own home and prevent hospitalization

Services include:

  • An in home assessment by our Heart Failure RN Coordinator followed by telephonic follow up for 30 days
  • Tasks performed during the home visit:
  • Explanation of Green, Yellow and Red Zone Symptoms
  • Medication Reconciliation
  • Scheduling Appointment with PCP
  • Instructions on Patient Self-Management
  • Functional Assessment
  • Nutritional Counseling
  • Coaching
