Comprehensive Care at Every Stage of Heart Failure

If you or someone in your family suffers from heart failure, the Torrance Memorial Medical Center Heart Failure Program can help. The Heart Failure Program is a comprehensive program that coordinates services and provides the special care needs of heart failure patients and their families in the South Bay area.

The goals of the program are to reduce the rates of hospital readmissions, decrease symptoms, and improve functioning and overall quality of life.

Services are provided on both an inpatient and outpatient basis and include evaluation and treatment of heart failure by a multidis- ciplinary team including cardiologists, surgeons, nurses, dietitians, pharmacists, and social workers to coordinate services and develop an individualized care plan.

This team equips the patient and the family with the resources needed to transition safely back home and provides education and support in heart failure and medication management using person- alized education, resource materials, and support groups.

The treatment plans are based on American Heart Association/ American College of Cardiology Get With the Guidelines-Heart Failure program for heart failure patients. Torrance Memorial’s heart failure care received the 2012 Get With the Guidelines Gold quality Award. This signifies that it has reached an aggressive goal of treating heart failure patients with 85 percent compliance for at least 24 consecutive months to core standard levels of care under these guidelines.
