South Bay Survivorship Consortium

The South Bay Survivorship Consortium was formed in 2009 to create a forum where healthcare providers can focus on assuring excellence in survivorship care in our community.

The Consortium's model of coordinated support strives to bring cancer survivors and their loved ones closer to the resources that will give them strength as they complete their cancer treatment and transition back to living their lives to the fullest.

Mission

Our mission is to improve the quality of care delivered to cancer survivors in the South Bay Community by providing education and support for cancer survivors and health care providers through the activities of a collaborative partnership of community-based healthcare organizations.

We believe survivorship begins the day of diagnosis for the person diagnosed and his/her loved ones. Survivorship issues should be addressed across the continuum of cancer, from diagnosis through end of life care.

Goals

  • Education of cancer survivors, Primary Care Providers and Oncology Specialists
  • Resource development and implementation to facilitate delivery of high quality, cost effective and appropriate care for cancer survivors
  • Communication of established standards of care for cancer survivorship to survivors and health care professionals
