Taking Care of Your Needs
We have a case management staff, consisting of registered nurses and trained case managers, who work with patients and family members to help insure that your time in the hospital is productive and to help you get what you need for a safe recovery at home.
In order to assess your needs, the case management team will ask you a series of questions, including:
- Do you live alone?
- Will someone be there to assist in your care after hospital discharge?
- Are you planning on a home care agency prior to returning home?
- What equipment do you already have?
Our case management team will help evaluate your needs, discuss resources and benefits that are or may be available to you, answer important questions about your hospital stay and assist with your discharge plans.
