  • Hourly: 1X annual salary.
  • Salaried: 2X annual salary.

Group Term Supplemental Life Insurance Employee Regular FT employees First of month following 30 days of employment Employees: up to 5x annual salary. Spouse: up to 50% of employee's coverage. Children: up to $10,000.
Insurance Liability Hospital All employees Upon employment Coverage for professional liability for actions within the scope of employment.
Universal Life with Long-Term Care Insurance Employee Regular FT and PT employees First of the month following 30 days of employment No minimum. Maximum: $300,000. Pays living benefit of up to 75% upon diagnosis of a terminal illness. Home Health and Long-Term Care rider pays up to two times the face amount in monthly benefits.
Critical Illness Insurance Plan Employee Regular FT and PT employees First of month following 30 days of employment Lump sum (from $10,000 to $100,000, not to exceed 2X salary, depending on option selected) paid the first time you or covered family member are diagnosed with a covered illness or condition.
Voluntary Short-Term and Long-Term Disability Employee Regular FT and PT employees First of month following 30 days of employment

Short Term: 30% of regular monthly earnings after 30 days, not to exceed weekly max of $1,400 for 47 weeks.

Long Term: after 360 days, 60% of monthly salary, max benefit $5,000, max benefit period of 5 years.

Short - OR Long-Term Disability Hospital Regular FT Salaried employees First of month following 30 days of employment Up to 66 2/3% of salary after 30 days of disability for 22 weeks, weekly max $3,845. Long Term Disability, 180 days waitng period, monthly max $16,665.
Voluntary Cancer Insurance Plan Employee Regular FT & PT employees First of month following 30 days of employment Pays inpatient and outpatient benefits;
Transportation & Lodging benefits;
Pay in addition to other coverage.
